Our Long-Term Care nursing facility is seeking for motivated and compassionate Licensed Practical Nurses to join our growing staff and provide direct care to residents while promoting the highest level of functioning. We are currently seeking Part-Time and Full-Time LPNs for ALL Shifts (7am-3pm, 3pm-11pm, 11pm-7am) Requirements Preferred to have experience in a Long Term Care environment, but new grads are welcome! Must possess and maintain a current NYS state license as a Licensed Practical Nurse (LPN). Must be able to move, lift and position patients. Knowledge of Sigma Care is Preferred Job Types: Part-time Responsibility: Monitoring residents, their responses to treatment care plans. Supervising and directing. Document necessary information regarding nursing care, care plans and observation of residents overall conditions. Perform various duties to provide quality nursing care to residents. Analyzes information for appropriate interventions to prevent decline. Record signs of change in condition. Notify appropriate staff per facility protocol to evaluate weight loss, a decline in ambulation, development of skin breakdown, etc. Administers medications and treatments to residents; monitor administration of intravenous fluids; orders places for medication. Maintain resident clinical files; keep charts updated; document appropriately. Make rounds to see each resident daily.

Responsibility: Direct the day-to-day functions of the nursing assistants in accordance with current rules, regulations, and guidelines that govern the long-term care facility. Ensure that the written policies and procedures that govern the day-to-day functions of the nursing service department are followed by all nursing personnel assigned to you. Ensure that the Nursing Service Procedures Manual is followed in rendering nursing care. Cooperate with other resident services when coordinating nursing services to ensure that the resident's total regimen of care is maintained. Perform administrative duties such as completing medical forms, reports, evaluations, charting, etc., as necessary. Participate in the facility's quality assurance program for the nursing service department. Complete the nursing services portion of the resident's discharge plan as necessary. Monitor nursing care to ensure that all residents are treated fairly, and with kindness, dignity, and respect. Provide data to the Quality Assurance & Assessment Committee as required. Prefer but not required, one year experience in a Nursing Home health-related facility. Salary is based on experience.
