As a Home Health Aide your essential duties are as follows: Accompany patients to their doctor visits, carry charts, take notes, and push wheelchair. Assist with patient's personal care needs, ie personal hygiene, etc. Document services provided in an accurate and timely manner in accordance with company policy and personal care plan. Maintain patient confidentiality at all times. Report any changes in the patient's condition or incidents to RN Supervisor or therapist in a timely manner. Carry out other duties that may be assigned Qualifications: MUST have current HHA certificate Work hour: Monday-Friday, irregular hours depend on the clinic trips, weekends/ holidays.
