Service Coordinator Position – Crown Heights
A growing healthcare organization in Crown Heights
is seeking a Service Coordinator
Key Responsibilities:
• Handle new patient intakes
and follow-ups
• Ensure all monthly and yearly
documentation is completed
accurately and on time
• Coordinate transportation and
order medical supplies
• Conduct quarterly in-person
visits with patients to assess
needs and maintain relationships
• Maintain regular office hours (9–5)
Ideal Qualifications:
• Associate's (or Bachelor's) degree
• Bilingual - English/Russian
• Strong organizational, communication, and
problem-solving skills
• Proficiency in basic computer skills, including e-mail,
documentation and data entry
• Experience in case management
or patient coordination is a plus
Salary: Starting at $40k–$50k/year
