Verizon New York Inc Lifeline service in New York:
Lifeline is a government assistance program that is supported by programs created by the New York Public Service Commission and the Federal Communications Commission. Verizon New York Inc. (Verizon) offers the following Lifeline services as an eligible carrier:
• Lifeline service at message rate: $1.00 per month
plus regular rates for each call made or
requested additional service.
• Flat Rate Lifeline: $2.00 per month plus monthly
$7.20 local use fee.
• Broadband service (Fios Internet) Lifeline:
discount $9.25 per month
In addition, Verizon does not charge Lifeline customers a federal subscription fee for the line.
Only those who are eligible can participate in the program. You may qualify for Lifeline if you can demonstrate that you meet one of the following requirements:
• You are participating in at least one of the programs listed below:
• Low Income Home Energy Assistance Program (LIHEAP), Medicaid, Supplemental Security Income (SSI), Supplemental Nutrition Assistance Program (SNAP, formerly known as Food Stamps Program), Federal Public Housing Assistance Program (Section 8) , Free School Lunch Program (NSLP), Temporary Assistance for Needy Families (TANF), Veterans Retirement Benefit, Veterans Survivor's Retirement Benefit.
• For residents of federally recognized tribal lands only: Bureau of Indian Affairs (BIA) General Assistance Program, Temporary Assistance for Needy Families (TTANF), Food Distribution Program on Indian Reservations (FDPIR), Head Start.
• You have an annual gross household income of or
below 135% of the federal poverty guideline.
Residents of federally recognized tribal lands eligible for Lifeline can receive up to $100 in Link-Up installation credits to connect to the service.
Verizon also offers a monthly Lifeline discount to qualified customers who sign up for Fios internet service. Eligible customers receive a discount of $9.25 per month.
In addition, Lifeline is limited to one discount per household on wired, wireless or broadband (Internet) service. Therefore, you must acknowledge and agree that no other household member receives Lifeline services from Verizon or any other carrier.
The Lifeline service is a non-transferable benefit. Consumers who knowingly make false statements in order to obtain a benefit may be subject to fines or imprisonment and may not be eligible to participate in the program.
To be eligible for Lifeline, you must meet certain requirements in New York. An application for Verizon Lifeline service can be obtained by contacting Verizon at www.verizon.com/lifeline or by calling 1-800-VERIZON
