Verizon New York Inc. Lifeline Service in New York
Lifeline is a government assistance program supported by programs created by the New York State Public Service Commission and the Federal Communications Commission. Verizon New York Inc. (Verizon) offers the following services supported by the Lifeline program as an eligible carrier:
• Lifeline service with message rate: $1.00 per month plus standard rates for each call made or additional service ordered.
•Lifeline Flat Rate Service: $2.00 per month plus $7.20 monthly local call fee. •Lifeline with Broadband Internet (Fios Internet): $9.25 monthly discount.
Additionally, Verizon does not charge the federal line fee to Lifeline customers.
Only eligible consumers can participate in the program. You may be eligible for Lifeline if you can demonstrate that you meet one of the following requirements:
•You participate in at least one of the programs listed below:
o Low-Income Home Energy Assistance Program (LIHEAP), Medicaid, Supplemental Security Income (SSI), Supplemental Nutrition Assistance Program (SNAP, formerly known as the Food Stamp Program), Federal Housing Assistance (Section 8), National School Lunch Program (NSLP), Temporary Assistance to Needy Families (TANF), Veterans' Retirement Benefits, Veterans' Families' Retirement Benefits.
o For residents of federally recognized tribal lands only: Bureau of Indian Affairs (BIA) General Assistance, Tribally Administered Temporary Assistance to Needy Families (TTANF), Food Distribution Program on Indian Reservations (FDPIR), Head Start.
•Your annual gross family income does not exceed 135% of the federal poverty level.
Residents living on federally recognized tribal lands who are eligible for the Lifeline program can receive up to $100 in credits toward the cost of installing a Link-Up to connect to the service.
Verizon also offers a monthly Lifeline discount for qualified customers who sign up for Fios Internet service. Qualified customers will receive a discount of $9.25 per month.
The Lifeline program is also limited to one discount on wireline, wireless, or broadband (Internet) per household. Therefore, you must acknowledge and agree that no other member of the household receives Lifeline service from Verizon or another carrier.
Lifeline is a non-transferable benefit. Consumers who knowingly make false statements to obtain a benefit may be subject to fines or imprisonment, or may be disqualified from the program.
