THIS FORM CONTAINS QUESTIONS YOU SHOULD ASK YOUR ANIMAL’S NEW OWNERS AND IT IS ADVISABLE TO ASK THEM TO FILL OUT THIS FORM. DO NOT GIVE YOUR ANIMALS JUST TO GET RID OF THEM!!!
