Service Coordinator - Healthcare Agency - Crown Heights/Brooklyn

A growing healthcare organization in Crown Heights/Brooklyn is seeking a Service Coordinator. Key Responsibilities: - Handle new patient intakes and follow-ups. - Ensure all monthly and yearly documentation is completed accurately and on time. - Coordinate transportation and order medical supplies. - Conduct quarterly in-person visits with patients to assess needs and maintain relationships. - Maintain regular office hours (9–5). Ideal Qualifications: - Associate's (or Bachelor's) degree. - Bilingual - English/Russian. - Strong organizational, communication, and problem-solving skills. - Proficiency in basic computer skills, including email, documentation, and data entry. - Experience in case management or patient coordination is a plus.
